Most companies use a sprayer (not a fogging machine) to try to sanitize the duct system but the sprayers are not effective to treat the entire duct system.

A comprehensive duct cleaning takes between 3-5 hours, depending upon the size of your house (and much longer for commercial applications). Most companies, especially the ones that have a price that seems to be “to good to be true”, are in and out of your house in less than an hour and may try to upsell you based on “mold” they allegedly find in your air handler.
Air duct cleaning is done by heating, ventilation and air conditioning (HVAC) professionals. The pros use industrial-strength, truck-mounted vacuums and powerful brushes and hoses to clean inside the metal ducts that make up your forced air heating and cooling system. The Environmental Protection Agency recommends duct cleaning if there is “substantial visible mold growth inside hard surface ducts, ducts that are infested with vermin such as rodents or insects, or ducts that are clogged with excessive amounts of dust and debris and/or particles are actually released into the home from your supply registers.”
A. First of all, be sure to be there when they do the job! We had our ducts cleaned and got robbed by one of the workers who thought we wouldn't miss a few "minor" personal items. But, even though they didn't believe us at first, we reported it to the police and the company and he got caught! Secondly, ask if they not only remove the vent covers but clean those too. Ask how they clean them and if they use their own cleaning supplies and take the vent covers outside to clean--not in your bathroom or kitchen sinks, using your personal towels. Ask if they clean up the lint that falls into the room with a vacuum cleaner. They should have a crew of at least two people to put the hose in and clean out the vents and one to clean the vent covers and the area around the vents where the lint falls. They should clean the air intake vents and cover also. Some try to sell you on cleaning out your actual AC/HVAC unit coils. I don't see the need for that, but if they insists it is needed, get them to show you the dirt in there that they will be cleaning out. If they can't show you, chances are they will not be able to get in there to clean it. They should change the air filter, but be sure they don't try to sell you some heavy-duty, life-time guarantee filter that supposedly never needs to be replaced. Those don't let enough air flow through. The medium grade ones changed every few months or so work the best. You can buy those at a home improvement store and change them out yourself during the year. Make sure that they don't damage your walls with their big machine, trying to get it down the hall and around corners. They damaged our walls, too and we had to have them plaster and paint it again. Just because the company has a nationally recognizeable name doesn't mean they will do the best job. Often these are owners who hire flunkies to do the work who are thieves and don't care. It would be better to get a local guy that has been doing it a long time and can furnish you with references of satisfied customers.
Make use of a business that will offer a reasonable quote, based upon the service that they will offer. Be tired of business that market one cost for an entire home. Consider it, would a painter paint any size home for one low price? Neither can a duct cleaner PROPERLY clean the entire home, no matter the number of systems or air vents remain in the house for one low price.

To be honest, the price was much higher than I expected or planned for. But I was impressed by the thorough attention to detail, even in the inspection made before quoting a price, and the professionalism and devotion to quality service that I could see in Mr. Stewart and his team. I suspect that others may have convinced me to add services for an additional cost that were included with his service, so I decided to go for the quality he offered and we were very happy with his detailed and thorough cleaning. The tobacco odor removal in a separate residence was the only thing that didn't seem to be as effective as expected but he came back to try again so I appreciate that. Overall, we're very happy with the cleaning and highly recommend this company.
Sergio and helper arrived promptly, explained everything they were going to do, very knowledgeable about my issues with mold/mildew and after examining AC air handler inside, explained what should be done to prevent mold/mildew from forming again. Had them also treat/fog the vents. They did a very good thorough job and vacuumed up all debris that came out of vents.
How often you should clean your air ducts depends on your situation. If you or someone in the home has asthma or is acutely allergic to certain airborne materials or pollen, regular duct cleaning may be helpful.
